<p class="MsoNormal"><span style="font-family:"APHont",sans-serif">Phase three of the covid vaccine is starting next week, which means all individuals with disabilities will be eligible. To facilitate the process, Heartland Community Health Center contacted
Independence, Inc. and extended the opportunity for us to assist Independence Inc. consumers schedule a time to receive the vaccine at Heartland Community Health Center, if you want to receive it. The next covid vaccine clinic at Heartland is Thursday, March
25<sup>th</sup>. (Consumers includes all past and present members of the Access Task Force who are individuals with a disability.)<o:p></o:p></span></p>

<p class="MsoNormal"><span style="font-family:"APHont",sans-serif">If you want to receive the covid vaccine at Heartland next Thursday, please reply to this email ASAP, no later than Monday noon, and include your phone number. I’ll then turn in your name, email
and phone number of those who want to receive the vaccine to our Executive Director Jill Dudley, who will turn in all the names to Heartland. Heartland will then email you a few forms to fill out, a way to confirm you want to receive the vaccine, and acknowledgement
you’ll be there at the time assigned by Heartland. <o:p></o:p></span></p>

<p class="MsoNormal"><span style="font-family:"APHont",sans-serif">Another option: If you want to pursue receiving the vaccine at the Douglas County Fairground drive thru clinic on Wednesday, March 24<sup>th</sup> You have to use the priority code in the fairgrounds
clinic attachment. There are only so many appts so so act on that asap since we cannot make any reservation for that clinic.<o:p></o:p></span></p>
