New MetroAccess Application

The Metro Office of Eligibility Certification has updated the MetroAccess application form for people with disabilities who are applying for paratransit service.  Attached is a PDF version of the new application, as well as a Word version of the new application.  The Metro Office of Eligibility Certification asks that individuals and organizations begin using the updated version of the application immediately, and discard/recycle any previous applications that individuals or agencies may have had on hand
