When you go to this website, there is a link 
To start or update an application. 
If you are recertifying, you should try the link recertification link. 
Make sure that when you fill out the form, after completing the first
part, name, address DOB and such, you will get a control ID number. 
Write it down and save it. If you have to call the office or go in
person, this number is helpful. 
If you have any other questions, please call me or write directly.
