[MD-Sligo] NFB Vaccination Booster Clinic - Sign Up by January 5, 2022 - Slots Going Fast!

Fellow Federationists,

The National Federation of the Blind and National Federation of the Blind of
Maryland, in partnership with the Maryland Department of Health, will host a
COVID-19 vaccination booster clinic on Friday, January 14, 2022.  The clinic
will take place at the National Federation of the Blind Jernigan Institute.
This clinic has been organized due to the inaccessibility of some online
vaccination scheduling platforms.  

 

The Department of Health will be furnishing all three vaccines, and
individuals may choose which one they prefer while supplies last.
Individuals 12 years and older are eligible, but age may impact which
vaccine an individual may be eligible to receive.

 

Those interested in receiving a vaccine must schedule an appointment in
advance.  They should contact both me (President at nfbmd.org
<mailto:President at nfbmd.org>  or 443-426-4110) and Tammy Helm (thelm at nfb.org
<mailto:thelm at nfb.org>  or 410-659-9314, Extension 2291) on or before
January 5, 2022 to request a vaccination slot.  Their appointment is not
scheduled until they receive confirmation.  Vaccination slots will be
scheduled on a first come first serve basis.  Appointment slots will be for
a thirty minute window sometime between 9 AM and 2 PM.  No walk-ins will be
accommodated.  Priority will be given to members and staff of the National
Federation of the Blind, individuals with disabilities, and those working in
or with the disability community.  Where slots remain available, those
eligible will be scheduled to receive the vaccine.

 

We are also seeking staff and volunteers to assist at the vaccine clinic.
NFB staff should contact Tammy Helm, and members who are not on staff should
contact me.  Please let us know by January 5, 2022 if you are able to work
the clinic.

 

Individuals receiving the vaccine should plan to bring copies of their
health insurance cards if they have coverage.  No one will be refused a
vaccine due to not having health insurance.  Individuals will also have to
review and sign a consent form.  We will try to share an electronic version
of the consent form with those whose vaccination appointments are scheduled
and confirmed, and human readers will be available to assist on site.  

 

Any requests for reasonable accommodations, other than assistance completing
vaccination consent forms, should be requested by January 5, 2022.  Such
