Care Program is designed to encourage individuals with severe chronic or
permanent physical disabilities to direct their attendant care services. The
Program provides financial reimbursement to supplement individuals' costs of
attendant care services. Services may include in-home assistance with personal
care, chore services, and transportation. The participant is reimbursed a
percentage of attendant care costs based on annual income, household size, and
available Program funding.<o:p></o:p></SPAN></P>
