<p>The AAHP, part of the County’s Department of Health and Human Services (DHHS), was started in 1999 to address health care disparities that disproportionally affect African Americans and people of African descent (the target population) in Montgomery County. AAHP’s vision is to ensure that all African Americans and people of African descent in Montgomery County are as healthy and safe as the rest of the population. Its mission is to eliminate health disparities and improve the number of years and quality of life for the target population in Montgomery County.</p>
<p>AAHP focuses on six major health areas: maternal and infant health; HIV/AIDS; diabetes; oral health; cardiovascular disease; and cancer. Aging, faith-based and mental health services are cross-cutting components. Services provided include outreach, health education, support groups, referrals, and nurse case management.
