I'm not sure what you mean about "proper translation", because most hymns
are translated poetically rather than literally. But here's a mostly literal
translation:

My Lord, Jesus divine,
Here I am in this hour.
To lift your name for ever
and give you praise.

Receive me, O Lord,
[and] what I offer to you.
Today I open my heart,
giving you worship.

Alleluia, Alleluia...
