In most cases I have found the admin side of themes to be accessible. That being said, I don’t want to deal with colors on a client’s site, so I’m working with a sighted Wordpress designer to modify the page to make it look sexier then what the original theme does. You can add your logo, and change most settings in appearance. NOTE, this is all coming from a self hosted point of view, not a Wordpress.com user’s point of view, I’ve never used Wordpress.com
