[nabop] Creating fillable forms in Microsoft word 2003

Hi,

    My name is Bryan Brown, I am a massage therapist in Wisconsin. I currently use a voice note to take all my Client information and health history forms. I'm worried about it breaking down. I'm wondering if there is any one who would be willing to help me figure out how to create a fillable form in Word? I hope someone can help me.
