NVDA uses Espeak as its synthesizer. It's not the best, but it definitely works and it's not bad at all for something that you can get for free.

FYI, I forwarded the email along to the other NFB-NET list I'm subscribed to, Blind Talk, and a few friends who use NVDA or have friends who do. I've heard the NVDA speech synthesizer before, and it has a pretty thick British accent. It almost sounds a little creepy to me, almost like a computer-voice Darth Vator, LOL. So, if your friend has trouble understanding some speech synthesizers, you might want to install Eloquence (if you can do that) or something.
