> I, Ashley Cwikla, Ph.D. Candidate from the Department of Leadership at the University of the Cumberlands invite you to participate in a research project entitled Self Determination, Attitude, and Satisfaction with Postsecondary Academic Accommodations for Students with Visual Impairments.
> The purpose of this research project is to determine if self-determination and attitude have an impact on requesting academic accommodation requests amongst students with visual impairments in a postsecondary educational setting. This study also aims to determine how satisfied students with visual impairments are with academic accommodations. Should you choose to participate, you will be asked to respond to three surveys pertaining to self-determination, attitude toward requesting academic accommodations in postsecondary education, and a disability accommodation satisfaction survey.

> If you choose to participate, please be sure to submit this survey no later than April 1.

> The expected duration of these survey responses should take less than an hour of your time. All survey responses will be kept confidential and no personal identifying information will be used during analysis or reporting of the results. Email addresses will be collected as part of the drawing after all responses have been received and will be subsequently deleted.
