The GDUI guide dog survey has those numbers.  You'd have to do the math to 
get the percentages.  The thing I found the most interesting from that 
survey was the number of hours that the various programs spend actually 
training the dog.  I thought it was all pretty much the same, but I was 
terribly wrong.  There is quite a large margain of difference.  Here's the

It's from 2006 so pretty recent.
