You can check out the details for the requirements for traveling to the UK
with your guide dog at:

Basically, in this order,  your dog must be microchipped, vaccinated against
rabies and satisfactorily blood tested. Great that you're starting to plan
now as you must wait six months from the date the blood sample that gave a
satisfactory test result was taken before you can enter the UK. 

A month or two before you travel, you will also need to get an official
third country veterinary certificate. In Canada, the certificate is issued
or endorsed by the  Canadian Food Inspection Agency (CFIA) Veterinary
Inspector.  There is a fee for stamping/endorsement. You can look up your
area office at:

Finally, within 24-48 hours before you leave, your vet will need to treat
your dog against ticks/tapeworm and record the treatment on your dog's
health certificate.

The Vet at Heathrow Airport is very helpful. She will work with you via
email or phone call if you have any questions and  if you fax details of
your health Certificate documentation in advance she will advise you of any
omissions which can be corrected prior to your departure. This also speeds
up the inspection process once you arrive in the UK. Her contact information
