That's a good idea.  I also wonder if it wouldn't be a good idea to
add some careful language explaining what the business owner can do.
At the moment, there's no mention of the business owners rights.  I
realize that that's not the purpose of the flier, but on the other
hand, a document which is too one-sided isn't likely to be well
received by some.

Here's my attempt at such language:
If you are not sure whether or not a dog is a guide or service dog,
you may ask the handler.  In order to be considered a service dog
under federal law, a dog must be trained to perform a specific task
for a person with a disability, and you may also ask the handler what
specific task the dog is trained to perform.  However, a handler is
not required to tell you about their disability, nor are they required
to have any sort of certification of their dog's training.
