Lora,

Here's what I think about each of your four scenarios.  I think I'm 
right, but I'm not a lawyer. *smile*

Yes to the dining room at the YMCA under normal dining circumstances.  
It would be like any other restaurant, college cafeteria or the like.

No to the kitchen.  Again under normal circumstances.  Now if the 
kitchen is open for public tours or something then yes.

No to surgery unless it is of the outpatient, in office, sort where you 
are not in a sterile environment and you can handle your dog during the 
procedure.  I'm thinking of something like dental surgery in a dentists 
office where you are awake and alert.

Yes to the dog shelter office.  It is a place where the public is invited.

There's also some differences if you are talking about an employment 
situation vs. a visiting member of the public.  Steve is excellent at 
this sort of thing.  I hope he will offer his thoughts.
