Yeah, I had that kind of thing happen to me at a medical clinic.  They had a 
12 year old record under my old legal name.  When I came back to that 
clinic, they found that old one and got confused because i was now under a 
new legal name.  So, yeah, I can see how that can confuse things - 
especially now they use computer records and not paper ones.

Glad the problem is solved.
