my best recommendation is to call freedom guide dogs ask to talk to Eric the co founder give him as much information about your situation as you can. I am not going to guarantee they will be  able to help you but this is most likely a good place to start. 
if you look at freedom's youtube channel there are some videos about some of the special cases that they have helped with
