I put one leg in, turn the dog so her back end is facing towards the
cab, have her back into the cab and then bring my other leg in. To get
out, I open the door, bring out one leg, have her hop out, swing out the
other leg. This takes some practice, but all of my dogs, including my 75
lb male GSD have figured this out and done it well. This way, tail goes
in first and has no chance of getting closed in the door, and the dog is
facing the right way when it is time to get out and has no reason to put
her paws on the seat at any point.

You'd end up getting out on the opposite side of where you entered and the dog would follow behind you. Of course this only works if you have the whole backseat to yourself.
