Hi Robert.
Are you perchance a GDB alum?  They had a teleconference on this very
subject, and have had alumni cruises to Hawaii.  They worked with a travel
agency near San Francisco specializing in working with disabled people. A
search of their website, or a conversation with grad services, if you're a
grad, might find these resources.
Cheryl Echevaria, president of the NFB Travel and Tourism Division, also
runs a travel agency specializing in working with disabled travelers.

I am writing, to ask, if anyone knows what the requirements might be in
order to cruise to Hawaii with your guide dog.
I am hoping to receive a new dog in October, and I am considering cruising
from Los Angeles to Hawaii in November on a two week cruise.
