I also noticed that the forms are accessible; however, I also echo Kevins opinions. It’s not the airlines business where we got our dogs, what our dogs name is, and what our dogs vets name is. Also, owner trainers are going to have serious issues when they need to fly.  it should only need to be a simple one paragraph statement that one has to agree to under perjury.
