[NAGDU] Flying guide / service dogs to Hawaii - APHIS paperwork

The EU is pretty straight forward and fairly easy. Hawaii is not. I know 
no matter how much I've tried, can not go to Iceland. I'm thinking 
England may be the same. Don't know for sure.
