Dear Tracy and Listers,

I would say Yes, fill it out in all cases when you travel with a guide or
service animal.
That way, we can get an idea of how often people are refused, or
discriminated against due to the presence of their dog.

I know Tim Elder encouraged us to keep filling out the NFB Uber service
animal survey, but are we supposed to fill it out in all cases, or only if
we had a problem?

It used to be in all cases, so they could determine the frequency of
problems, but is it different now?
