I have to take back some of my words. Here is a link that gives good information but it can be a little confusing. Religious organizations are covered under Title 3 of the ADA and also covered under 504.

Heather,
you gave a great explanation. Religious entities can refuse, except where they house a public entity such a s shelter, day care center, etc. that is open to the public.
I have personally had mixed responses to my guide dog within the church and on the church's transportation vehicle. People's personal cars are a different story. It is more difficult to get rides from individuals over the long haul. Just sharing my experiences.
