[NAGDU] Filling out American's Health and Behavior Form

Hi, all.

I'm trying to fill out a form on American Airlines web site in order to 
travel with my guide dog.  The form is in both PDF and Adobe Acrobat, 
I'm trying the PDF version.

The PDF form doesn't seem to have any way to access form fields, etc.  
I'm using NVDA 2022.1 and the latest version of Chrome.


What methods have you used to get this form filled out and submitted?


I got conflicting information from American when I made my reservation 
three weeks ago, so now I'm scrambling to get everything filled out on 
time.  <sigh>
