I just flew Southwest on 9/1 and 9/8. They did not ask for the form on
my departure from St. Louis, but they asked for it on my return from
Tucson.

I flew United back in February, and it was so easy. Once you make your
reservation, there's a link to the form. It's a nice, screen reader friendly
HTML form, no PDF, and if you have United mileage program, the form is good
for a year. My best friend just got tickets from southwest to come for a
visit, and apparently, they don't have an online form, asked him to fill it
out, print it out and take the paper copy with him to the airport. He
doesn't own a printer, so that's a pain. But several guide dog handlers I
know have flown Southwest recently and weren't asked for the forms at all. I
think all the airlines handle this differently, which makes the process more
confusing. Good luck!

Hi, all.
That time we all hate has come. I'm in the process of retiring my current
dog. I've already got a home for her, but it is several states away, so
flying is the only option.
I haven't flown since 2019, so the new rules and regulations weren't in
place. I simply booked a flight, had my ID and my dog's ID and a health
certificate and had no issues.
Now I know I need to fill out these numerous forms.
Which airline is less bureaucratic and easier to deal with? Also, how does
the paperwork go if purchasing through a site like Travelosity? Would it be
less stressful to purchase and deal directly with an airline? How are
situations handled if switching airlines? I mean flying on a commuter
airline from a smaller airport and then catching another flight on a larger,
different airline?
Depending on circumstances, this might happen as soon as late October, so I
guess that factors in to the mix, too.
