It is one we all dread. I recently flew and this is my experience booking online. The form process is simple and can be used on any airline in the US. One is am attestation, the other asks for information about your service dog.Fill them out, sign them, and take them with the health certificate to the ticket counter. The agent will validate your forms and will note it in the system.  If you book online they may give you a revised ticket noting you have a service dog .They will return you paperwork so keep them with you.
