[NAGDU] Really need more surveys completed

Just sent mine in. 

There’s one question, or a series of questions, depending, that I didn’t find there. 

Do you limit your travel based on having to complete and submit documentation for your guide dog? 

Sorry, I know that’s not very helpful at this stage. I am sharing the survey though.
