[NAGDU] United Airlines service dog form

Hi Melissa. 
Yep, that's where I was, but it only opens as PDF, and is very hard to fill
out.
I went to the United site, put in my confirmation number, found my trip, and
saw that I am marked as "blind".  I didn't see any link having anything to
do with service animal forms. What am I missing?
If they're going to make this assinine requirement, the very least they
could do is make it easy to find. 
I suppose I could pay Aira to help me fill in the PDF, but I don't want to
pay for something I feel strongly is an imposition.

United is the airline I typically fly and my experience was that once I
booked my trip and disclosed my dog and put in my special service request,
the link to the form brought up an HTML version that I could complete and
then when I hit a button it generated a PDF. I found updating my form in my
mileage plus account was actually weirder than doing it the first time. I'd
be curious to know how you resolved this. It would be great if all airlines
had the same process but no such luck.

I thought this was supposed to be easy.  NOT!  And I'm fairly tech savvy. 

I Googled "United Airlines service dog form" and got to a web page with
links to the DOT forms.  They came up in PDF, and I downloaded them. 

I opened one, which for a while showed as blank, but then gave me fields to
fill out.  I can't review the fields to check spelling or whatever, nor can
I check the check boxes for Rabies shots etc. 

Am I supposed to convert it to Word or something? 

I am using the latest version of Jaws, and Adobe BC, or whatever it's
called.  

What do I do?

Really, I should get paid for using my precious time on this nonsense.
