The thing about using a form from a few months ago is if it is dated before you made your current reservation it might get flagged. This is if anybody is paying attention though.

Thanks.  With your pointer, I found the form, and I'll print it out, even though I already have one from a few months ago. 
Thanks Tina for the warning.  If people going to convention do get hassled, with the national leadership say even a mumbling word about it?  Or would they really prefer we give up this dog thing and use the sacred white cane like they do.

Your form should be in your frequent flier profile. What I've done in the past, just to be on the safe side, is go to update form and put the new date that I signed it and print it so I also have a copy with me. In my case though I also had to update the rabies info anyway. Hope that helps some.

I don't actually have a copy of the service animal form.  I think I filled it out on the United website, though it's been a while and I forget.
I do have the form I filled out for my trip to Ireland, on the same airline, earlier this year.  Should I bring that?  Am I actually a first-class citizen?
