Information about the Workshops/Classes

Chronic Disease Self-Management Program (Tool Kit for Active Living with
Chronic Conditions)

The Workshops are FREE (Diabetes, Chronic Conditions) and (1)they are one
 led by professionally trained Instructors(1) hour by phone each week for
six (6) weeks.

Diabetes Self-Management Program (Tool Kit for Active Living with Diabetes)

Six weeks 1 hour week completion is 4 out of 6 weeks participants will
receive a certificate after completion.

You will learn after the class to :

Work with medical provider health care team by asking the right questions

Complications management for example address low and high blood sugar

Promote strength through exercise

Increase knowledge of your health and meal planning

Each participant will receive:

A diabetic wellness kit with :

Audio CD for exercise and relaxation

Downloadable Audio Book

And more
