[NFB-Arkansas] Membership dues

Hello everyone. In the at large meeting we were informed by Terry (our
state president) that we need to complete a form to include with our dues
which need to be received by our treasurer by January 1, 2019.
State dues are $5.00 and if you belong to the at large chapter, dues are
$5.00 as well.
