I hold the old and new yarn for about three or four stitches. On the next round, when I reach the old tail, I'll pick it up and knit a couple of stitches before dropping it again. For in the round knitting, I might do this on 3 rows. For flat knitting, definitely 2 rows so the yarn is locked in, but sometimes 3 rows. I don't cut the ends until after the first wash.
