I'm not Marsha, but I can tell you how I do it.  
I have taught my dog to walk to the left of my SSP or terp so they can sign into my right hand while my left holds on to the harness.  
In really crowded situations, I will go sighted guide but instead of taking the elbow, I rest my hand on theirs so we can communicate, and if I have to go behind them, like in a line, I will rest my hand on their back so I can quickly move it back to their hand.  If we are shopping, I hold on to the cart and let Soleil walk beside me.
