Subject: FW: 2018 Needs Assessment Survey

Good morning everyone,

These are the 2018 Needs Assessment Survey that clients, family members can
fill out. Not all of the survey needs to be completed, but the IL
services/training received and how these services have benefited should be
noted. If you are helping complete the survey and mailing to SILC, please
keep a copy. They will accept email scan, fax, mail or hand delivered.

All hard copies due to SILC by 5:00 pm, November 7, 2018

It would be very important to have B/VI population attend the potential
public forums and provide input. I will send more detail when I receive
the final locations.

North & North-central Idaho

. Coeur d'Alene

. Orofino - Kamiah

. Cottonwood - Riggins - Grangeville

. Moscow

. Potential Disability Mentoring Day in one community

Eastern Idaho

. Pocatello

. Driggs (English & Spanish)

. Idaho Falls

. Burley

. Rexburg

. Potential Disability Mentoring Day in one community

South & South Central

. Boise, IL conference, September 7

. Boise (2, including refugee community)

. Nampa/Caldwell - English and Spanish

. Twin Falls

. Twin Falls Disability Mentoring Day

. Middleton

. Hailey

. Magic Valley Transition School

. The CILs and the SILC will work together to arrange venues, obtain
supplies and provide advertising for each community with the CILs taking the
lead in most locations. SILC will provide all printed materials, a
facilitator and a note taker. CIL staff (usually the Director) will
facilitate the meetings with SILC staff. Some youth transition meetings will
be arranged and facilitated by the SILC staff. CIL staff may in participate
in youth meetings as their resources allow.

. During the last statewide assessment process, we had ASL
interpreters at all public meetings. We only used them in two locations:
Boise and Orofino. We plan to only get ASL interpreters upon request or in
areas where we anticipate participation. This process allows Idaho's
limited number of ASL interpreters who are available to provide services
where they are most needed, rather than sitting through a meeting where
their skills aren't used.
