Good morning, Dana,
Below is info I received for my job but it directly related to us and our
ability to have input into the data collected regarding COVID immunizations.
Can you please pass this on to our entire membership and everyone you may
know ASAP? They have extended the deadline until this Friday but that is
only a few days from now. Thanks! -Erin

The American Association on Health and Disability has received a very large
response to our COVID-19 and Vaccine Survey for Adults with Disabilities.
But we're missing some important voices. We are extending the deadline to
complete the survey for one week and especially encourage adults with vision
or hearing disabilities, persons with disabilities who are Hispanic, Latino,
or of Spanish origin, men, and persons 65+ to complete the survey. The
survey uses disability identifiers from the American Community Survey and
asks a series of disability-specific and general questions about vaccine
decisions.

The average completion time for the survey is less than 4 minutes.
Alternate formats such as large print, Braille, or electronic versions of
