To my understanding, there is some sand in it to help it from roling over. I also think your weight helps it to stay in one place. People do things with their hands and bounce on the ball. Your bottom sourt of sinks into the ball a bit, so there is not any pressure on your spine, like in a chair.
I am sure others can describe it better. It takes pressure of like disks.
