I also use my thumb and forefinger to hold the last chain that I have made.  This keeps some tension on the loop making it easier to get the hook through the loop.

I do hold the yarn a little different then described by Theresa 
 
 
Tail of yarn comes in front of little finger going toward back of ring  
finger then up and coming forward over back of pointer finger [if you are left  
handed]it is coming on the left side of your pointer finger toward the palm 
side  of your hand.and then that is the yarn that is going to your slip 
kknot  loop.  I keep the tension on the yarn by holding my pointer up and once 
you  get a couple of chain stitches going, I hold the chain between my thumb 
and  middle finger on my left hand.   Whenever you get a row or that I  
hold the work with my left thumb and middle finger and my pointer is held 
upward  to form a side ways
