Hi Joyce,
To send a text message by voice, you hold down the big round button  
at the bottom until it beeps. Then you say, "Send a text message to  
<say phone number here>, then it'll ask you what you want to say. You  
hold the home button down again and speak the message content. Then  
it'll ask you if you're ready to send it. Hold the big round button  
down one more time and say, "Send." That should send your message.
You can actually use the notes app by voice too. You just say, "Note  
that ..." followed by the information you want to take note of.
To move from item to item on the screen, you have a couple options:
1) You can just move your finger around the screen and listen to what  
comes up.
2) You can flick with your finger to the right or left, and the  
iPhone will move forward or backward through the items on the screen.
3) You can also scroll through pages of list items etc. by flicking  
up, down, right or left with 3 fingers. If you 3-finger-flick up, the  
current contents of the screen will move upward off the screen and  
new material will move up onto the screen from below; the reverse  
happens if you 3-finger-flick down. If you 3-finger-flick left, the  
current contents of the screen move off the screen to the left and  
new content comes left onto the screen from the right. The reverse of  
this is true if you 3-finger-flick right. More often than not,  
though, you'll use the up and down scrolls, though left and right do  
come in handy on some web pages.
If you want to move straight to the first item on the screen, you can  
tap with 4 finggers at the top of the screen. Do the same at the  
bottom to go to the last item.
Once you've found something you want to activate, you double-tap.  
Note this doesn't have to be done in the exact spot on the screen  
where the item is located, you can do that anywhere once you have  
something selected.
