Just saw this.  I have a pdf copy of the following book.  I copied and
pasted the info below from my copy.  Do you have this one?  If you want it,
I can send it to you as an attachment.

All I did was put the book title in google without the author.  It of
coarse, took me to Amazon.  I did not go further.
