You could use 1/8  or 1/4 inch wide ribbon to fashion a mouth and attach it with sewing needle and thread.

Hi, 

I hope this request is an appropriate one to send to the list. I certainly 
do not want to do anything that would be considered inappropriate. I have 
sewn some teddy bears to go in the operation Christmas child boxes. I have 
them all done except the face and stuffing. The bears are 6 inches tall I 
was wondering if I could get some suggestions on what I might use for 
their mouths? I thought about using sequence, but they are just a little to 
small for my hands to use any more. I am sewing a smiley face shank button 
on the chest where the heart would be. thanks in advance for any 
suggestions.
