Good morning, 
 
We are inviting you to participate in a vital survey titled, "Understanding Low Vision Practice Patterns and Barriers to Care in the United States," conducted by investigators at UIC affiliated with The Chicago Lighthouse. This study aims to better understand current practice patterns of low vision care and identify the needs and challenges amongst low vision care providers in the United States. This survey will take approximately 20 minutes to complete and will ask you questions about your practice, how you manage patient care and billing, as well as perceived barriers to providing and receiving low vision services. Your responses will help us gain valuable insights about ways to improve the practice of low vision rehabilitation with the goal of improving care nationwide. 

To thank you for your time, after completing the survey, you can enter a random drawing to receive one of ten $25 gift cards. We expect approximately 200 people to complete the survey. The risks of participation are minimal and involve risks similar to a person’s everyday use of the Internet. Results will be shared collectively without identifying any individual participant's data.
