Attached is the pre-registration information and print version of the form for anyone who needs it. Please note that when you submit your payment through PayPal, a new webpage does not load, but a message will appear under the “submit close quote button telling you that your payment has been successful. If you then click on the “return to merchant” button, you will get an error message, but don’t worry: your registration has been processed.
