That's actually not hard at all.  The only reason so many apps are 
not accessible out of the box is that they use custom views that 
require the programmer to implement a method to define what VoiceOver 
should say.  They don't care to do it, so the controls say nothing.

It's the equivalent of putting an alt tag on an image, and it's just 
about that difficult to do.  In other words, the programmer needs 
only to want it done and take a few minutes to make it happen.  And 
yet the developers of a great many applications simply cannot be 
bothered to do it.
