Hello Tyler,

Thanks for the information you provided me. It's given me an idea where to
start.  It does look like I'll have to go to an outside  service to ensure
my forms do comply with HIPAA.  The cost to become compliant seems to be
too  prehibitive.

I'm glad to see you've had a free offer of help, but I'll do whatever I
can to help as well.

First, I want to get into some of the details of HIPAA compliance as this
will be very important to you. You'll need to have a host which is HIPAA
compliant in the least in order to do this. If you are hosting your own
servers, you will need to insure that they are compliant via the
evaluation standards. Unlike PCI compliance however, there's no governing
body apart from the HSS. So in the least you'll need to make sure that the
hosting environment is compliant. You'll also need to read these standards
to insure that you have everything, but you'll need to start with an SSL
certificate, insuring that the content is never publicly available
(protected via user login), insuring that those who administrate this
information. Last but not least, you'll need to look at caching and other
procedures to insure that the data stays protected.
This is something that you can probably dive into and I'm happy to
recommend where needed if you have specific questions. I would suggest,
however that you offload your form entry to another company and/or service
which you've vetted and can insure compliance. It would save you a great
deal of money and would mean less work for you in the longrun.
