One form generation solution, that is accessible, is Gravity Forms. NFB
Illinois uses them and the forms generated are accessible. You do need to
pay a yearly fee for a license.

I would caution you about using a WordPress solution as this will depend
on whether your WordPress installation and Web server are secure, use
encrypted connections, and other requirements to comply with your
requested HIPAA compliance.

You will probably be better off using a different solution such as Google
forms as I believe blind Inc has Google apps which is FERPA compliant. If
you really need online forms to be HIPAA compliant, you may be forced to
go with a separate paid solution.
