[Nfb-web] displaying php /html file

Hi,

 

Could someone think of why a php/html file will not completely load?

I have many text fields, two combo lists, a comment block and submit button.

The format is the same throughout but the comment text area, submit button
and footer lines will not show up.

I've tried a combination of php and html and straight php and this happens
from both files.
