Canadian Federation of the blind uses google forms.  Survey creation and 
execution is accessible except that on some older browser and screen reader 
combinations it doesn't seem to work well.

There is embed code that you can drop straight into a wordpress page or 
article to inject your survey directly into your wordpress or drupal site 
or any cms that will let you directly embed code.

There are very basic analytics on the web back end.  You can also port 
results from a google sheet into sheets or excel and run advanced 
statistical analysis if you want.

It's free.

The only drawback is that some people are unreasonably paranoyed about 
sending and receiving information through google.
