The below research participant solicitation is being provided for
informational purposes only. The National Federation of the Blind has no
involvement in this research, but we believe that it may contribute to our
research mission.

Hi all!

We are asking blind and low-vision adults to participate in a research
project entitled “Socioeconomic Disparities in Adaptive Sports.” The
purpose of this study is to determine if barriers to adaptive sports
participation disproportionately affect people based on socioeconomic
factors. You are being asked to complete an anonymous questionnaire about
your disability, socioeconomic factors, and your opinions regarding
adaptive sports participation. The survey should take approximately
5 minutes.

Please follow this link to our survey:

https://mcwisc.co1.qualtrics.com/jfe/form/SV_6VRBPI9jsXvVIRU

The survey will be completed electronically using Qualtrix software. No
private or individually identifying information will be collected and
participation will be anonymous. There are no risks to participating in
this study. Only researchers who are involved in this study, members of the
Medical College of Wisconsin Institutional Review Board (IRB), and auditors
from regulatory agencies will have access to these questions.

The information that is obtained from this study may be useful and may
possibly be helpful to others. There will be no direct benefits or
compensation to you.

This is 100% voluntary and you do not have to participate in this study.
You can withdraw from this study at any time for any reason without penalty.

Thank you for your consideration. If you have any questions or concerns
regarding this study, please call (414)-368-0620 or email
adaptivesportssurvey at mcw.edu .

Thank you!

The National Federation of the Blind knows that blindness is not the
characteristic that defines you or your future. Every day we raise the
expectations of blind people, because low expectations create obstacles
between blind people and our dreams. You can live the life you want;
blindness is not what holds you back.
