Hi,
Thanks for your note.

No one has been able to help me successfully because Amz won't Give us an alternative. Carmen and I really tried!  She was able to do hers but I cannot figure out how to do it. 

I have a pdf picture of my card, but I'm lost. I do not know how to upload.

I am totally blind.
