[NFBC-At-Large] uploading picture on Amazon

Hi,
Disabled people are able to have Amazon Prime 50% off. However, they need an uploaded picture of one's card. I do not know how to do that. Can anyone help me? Is there an amazon person to talk to who could help with this?
